A. AID TO INDIVIDUAL IN CRISIS SITUATION (AICS)
ABOUT THE SERVICE:
This service aids the needs of the families/individual who are in crisis situation and comes to the office or referred to avail for assistance in order to cope up with his present problems due to socio-economic difficulties and eventually to attain normal functioning.
REQUIREMENTS:
(Provision of Medical Assistance)
- Barangay Certification signed by the Barangay Chairman certifying that the bearer is really in dire need of assistance and belongs to the indigent/low income family.
- Medical Certificate from the Hospital where the sick person was confined.
- Form 200 from our office (MSWDO) duly signed by the client to be accomplished by the Social Worker.
(Provision of Burial Assistance)
- Barangay Certification signed by the Barangay Chairman certifying that the bearer is really in dire need of assistance and belongs to the indigent/low income family.
- Death Certificate of the Deceased Person
- Form 200 from our office (MSWDO) duly signed by the client to be accomplished by the Social Worker.

This service provides emergency shelter assistance to fire and typhoon, earthquake and erosion victims thru provision of limited financial and material assistance to help the victims reconstruct/repair their houses which were totally destroyed.

G. ISSUANCE OF SOLO PARENT IDENTIFICATION CARD
ABOUT THE SERVICE:
RA 8972 – an act providing benefits and privileges to solo parents and their children, appropriating funds therefore and for other purposes

REQUIREMENTS:
- Barangay certification certifying Solo Parent’s residency in the barangay for the last six (6) months
- Affidavit of circumstance stating the reason for becoming a solo parent (Note: Some LGUs may require only basic documents hence, it is advisable to call the C/MSWDO first to check the required documents
- Certificates e.g. birth certificate of children, death certificate of spouse (if applicable) and other appropriate documentary requirements
- Income tax return or any document that will establish the income level of the solo parent
- 1 x 1 picture ID (Note: ID is renewable every three (3) years.)
