This office is responsible for the implementation of R. A. 3753 or the Civil Registry Law which mandates the establishment of a civil register in the Philippines where acts, events (birth, marriage, death), court decrees (adoption, annulment, correction of entries, etc) and legal instruments (legitimation, acknowledgement, R. A. 9255, R. A. 9048) that affect the civil status of an individual and shall be properly recorded.

A. ISSUANCE OF MARRIAGE LICENSE
ABOUT THE SERVICE:
Before getting married each of the contracting parties must file separate sworn applications for Marriage License with the proper local civil registrar of the place where either or both of the contracting parties reside.
Marriage licenses are valid in any part of the Philippines for a period of 120 days from the date of issue. They are deemed automatically cancelled if the contracting parties have not yet gotten married within this period.
REQUIREMENTS:
- Original or certified true copies of Birth Certificates or in the absence thereof, the baptismal certificates of the contracting parties, otherwise the personal appearance of the parents of both parties to attest to the ages of the applicants is necessary;
- Proof of termination of previous marriage, if any, such as death certificate, decree of presumptive death and annulment decree of marriage, as the case may be;
- Applicants 18-21 years of age – consent in writing by the father/mother/surviving parent/guardian who must appear in person to the Municipal Civil Registrar or execute an affidavit to be attested by any official authorized by law to administer oaths, to be recorded together with the Application for Marriage License;

B. REGISTRATION OF MARRIAGE

For ordinary marriages, the time for submission of the Certificate of Marriage is 15 days following the solemnization of marriage. For marriages exempt from the license requirement, the prescribed period is 30 days.

D. REGISTRATION OF BIRTH
ABOUT THE SERVICE:
The birth of a child, being a vital event for a person, must be registered within 30 days from the time of birth at the office of the Civil Registrar of the city/municipality where the birth occurred.
The declaration of the physician or midwife or “hilot” in attendance at the birth or the declaration of either parent of the newborn child and the presentation of a marriage contract in cases of married parents shall be sufficient for the registration of a birth in the civil register.
